Nuestras Raíces, Inc

Urban Food Agriculture

Nuestras Raíces, Inc. is an urban agricultural organization located at 329 Main Street in Holyoke, Massachusetts, operating a five-acre farm and serving as the lead grantee for Nuestros Productos, a culturally responsive produce prescription program funded by a USDA Gus Schumacher Produce Prescription Grant. In partnership with the Holyoke Health Center, Community Care Cooperative (C3), and the UMass Center for Program Evaluation, Nuestras Raíces designed and implemented the six-week program to address food insecurity, improve dietary behaviors, and support cardiometabolic health among Hispanic/Latinx adults with prediabetes or type 2 diabetes who held C3 Medicaid or Medicare coverage. Across three cohorts from July 2023 to March 2025, the program enrolled 103 participants — predominantly older, low-income, Spanish-speaking women living at or below the Federal Poverty Guideline — who received biweekly produce boxes (up to 36 lbs. per household), bilingual nutrition education classes led by Darwin Cruz (Nuestras Raíces’ nurse and diabetes educator), and $20 gift vouchers redeemable at Nuestras Raíces’ farm or local farmers’ markets. While the program yielded marginally significant improvements in dietary behaviors (particularly reduced consumption of fried foods and white rice/bread, and better portion control), statistically significant changes in food security, nutrition security, and self-rated health were not achieved, largely due to challenges with recruitment through the health center, narrow eligibility criteria tied to C3 insurance, transportation barriers, and control group contamination within the small, tightly networked Holyoke community.

Holyoke Health Center

Federally Qualified Health Center

The Holyoke Health Center (HHC), a Federally Qualified Health Center (FQHC) located in Holyoke, Massachusetts, served as a key clinical partner in the Nuestros Productos produce prescription program alongside lead grantee, Nuestras Raíces. HHC’s primary role was to have its primary care providers identify and refer eligible Hispanic/Latinx patients with prediabetes or type 2 diabetes who carried C3 Medicaid or Medicare coverage into the program.

Public Health Institute for Western Massachusetts

The Public Health Institute of Western Massachusetts (PHIWM) is a Springfield-based nonprofit organization, located at 1350 Main Street, whose vision is for all people within the western Massachusetts region to have access to what they need to lead healthy lives. Working toward that vision, PHIWM builds measurably healthier and more equitable communities through community engagement, collaborative partnerships, research and evaluation, and policy advocacy, with a commitment to continuous learning, fostering collaboration, and amplifying the voices of those they serve. The organization’s core areas of work span several interconnected domains: coalition building — where they provide planning, facilitation, evaluation, and sustainability planning while prioritizing community ownership and health equity — alongside health policy advocacy, data analysis, and research that helps providers, policymakers, and communities target and measure the impact of their programs. Notable initiatives include the Pioneer Valley Asthma Coalition, a Youth Mental Health Roadmap for Western Massachusetts, and 413Cares.org, a free online community resource database connecting western Massachusetts residents to local services. PHIWM also serves as the regional data and action support provider for Western and Central Massachusetts under the Community Health Equity Initiative, and most recently released a regional Community Health Needs Assessment report synthesizing health findings across five counties to support more coordinated action across the region.Dr. Martínez served as an executive board member for 3 years. In 2023-2024 she was Vice President of the Board.
